Coordinating Supports and Services
To connect you with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals. We provide you Support Connection Service, Support Coordination Service and Specialist Support Coordination Services (if your situations are complex) by one of our qualified and skilled support staff. Our experienced support coordination team will help you in:

- Find service providers for supports you are funded for

- Access healthcare and education

Community Nursing (24/7 Complex Support, After Hospital Support)
Nursing Care (Complex Bowel Care, Enteral Feeding, Tracheostomy Management, Urinary Catheter and continence Management, Ventilator management, Subcutaneous Injections, Complex Bowel Management, Medication support and management, Diabetes management, Transition care program, Pain management)
Palliative Care (Cancer medication, Pain, Trouble sleeping, Shortness of breath, Loss of appetite, fatigue, Depression or sadness, Anxiety and nervousness, constipation, Anorexia, dementia)
After hospital Care (Care au provides Community nursing services to all transition patients including Cancer, Heart disease, Cystic fibrosis, dementia, end-stage liver disease, kidney failure, lung disease, Parkinson’s disease, stroke)
